A:

If there was only the cosmic microwave background radiation, then the loss of entropy due to cooling would just cancel the gain of entropy from expansion, just like in the quasi-static adiabatic expansion of a gas. The increase in entropy is due to other processes, including heating from gravitational collapse of stars, conversion of nuclear energy to thermal energy in stars, and thermal radiation from stars.
